Forms for St. Francis Xavier

The links found on this page are the forms to be used by the parish. All forms are stored in "pdf" format. If you do not have a reader for "pdf" files one can be obtained free from http://www.adobe.com/products/acrobat/readstep2.html .

Forms are to be printed out and returned to the church. You may drop off any completed forms at the church office during normal office hours or simply place a completed form into the collection basket while attending church services. Forms can not be accepted electronically at this time.

Parish Registration.pdf New members of the Parish should complete this form.
Volunteer Release.pdf This form must be completed by anyone who wishes to become an unpaid volunteer in the parish. Retention of this form shall be for seven (7) years following the termination of the service.
Volunteers Personal Profile.pdf This form is to be used when the volunteer position involves providing ministry to minors or vulnerable adults. Retention of this form shall be for seven (7) years following the termination of the service. In addition a Volunteer Release form must be completed.
Ministries.pdf Choose at least one Ministry and help us to build our parish. The more volunteers we have the better the parish will become and the less work for each of us.
